If you have ... Before sending Greirat to pillage Irythill, I ran into Patches and got him to stay at Firelink, saved Siegward and gave him his armor whilst buying another set off of Patches (NG+). Then AFTER I cleared Irythill up-to the Church of Yorshka bonfire I homeward bone'd back to the Shrine and told Greirat he is good to go.Basically, you need Siegward to be IN the kitchen to save Greirat. If you journey too far into the game, Siegward will continue his mission as well and Greirat will die. Specifically, if you go into the Dungeon then Siegward will be captured, and won't save Greirat.
